We are using a new online service called Register with a GP that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one. Please attend our surgery and ask at reception.
How to register as a new patient online
Complete the online form to register to become a patient at Coachmans Medical Practice.
You will need to fill in all of the details to register at our surgery.
We may ask you for details of your previous GP surgery and basic health and medical information.

You must have a Broadfield address to register with us. Please use the below catchment area map to check whether you are within our acceptance area.

Registering online as a non-English speaker
You can find guidance and advice on how to become a new patient and the NHS healthcare services you can access, if your first language is not English, or if you’re an asylum seeker on the Government website.
Go to the NHS entitlements migrant health page on the Government website.
Registering as a temporary patient
If you are away from home, you can register as a temporary patient with Coachmans Medical Practice for up to three months. This will allow you to use our surgery and still remain a patient at your permanent GP surgery. After three months, you will have to re-register as a temporary patient or permanently register with us.
You can register as a temporary patient at by visiting the surgery and completing the form.
Registering a child
Children under 16 need to be registered by their parent or guardian. You can do this by completing the online GP surgery registration form on their behalf. Parents or Guardians must be registered with us also. Newborns will be contacted upon registration to organise the 6-8 week mother and baby check and first immunisations.
Changing address
If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you may be asked to register with a surgery that’s closer to your new home address.
You can call the surgery or attend the surgery to complete a change of address form.
